Before you book
• Tell us the main reason for the appointment and any accessibility, interpreter, chaperone or communication needs.
• If you are requesting a fit note, bring relevant illness, work-role and absence-date details.
How to prepare
• Bring a list of medicines, allergies and major past medical history.
• Bring relevant letters, reports, blood results or discharge summaries.
• For children, bring a parent or person with parental responsibility wherever possible.

We will confirm your identity, explain the consultation and ask for consent before any examination. A chaperone can be offered for intimate or sensitive examinations.
